New Patients

We encourage new patients to arrive 15 minutes before their appointment time to complete paperwork. New patients may download the required new patient forms and fax them (972-548-0425) with a copy of the front and back of their insurance card.

Checklist before coming to the office:

• Do you have your current insurance card?
• If you have a new baby, has the child been added to your insurance?
• Do you know your benefit plan?
• Have you selected one of our physicians as your primary care physician (PCP)?
• Are you prepared to pay the co-pay, deductible, or previous balance?
• If you are a new patient, have you completed the New Patient Form (Adobe Acrobat Reader Required)?
• Immunization shot records?
• Medical records (if you have them on hand)?
• Have you requested your medical records from your previous physician? Release of Medical Records Form (Adobe Acrobat Reader Required)?
