Prescription Refills

When requesting a prescription to be refilled, please leave the following information at 972-548-0758 ext 245:

• SPELL the first and last name of the patient.
• State the date of birth.
• Give the name of the medication you are requesting.
• State the dose and amount of medication needed.
• Give the name and telephone number of the pharmacy you wish to fill the prescription.

If you are in need of a prescription to be filled for ADD or ADHD, please call 48 hours in advance so that we may have your prescription ready for you. All other prescriptions will be called into the pharmacy in the order they are received.

Sometimes, we may request your child to come into the office to be seen by our physician before a prescription can be refilled.
